I am glad to see the new press that chin implants have received in the news over the last few weeks. Chin implants have been one of my favorite procedures to perform over the last 10 years and I have done more than any surgeon that I know of over that period of time. Chin implants are the most effective way to improve someone’s self esteem, especially for those people who have microgenia or a weak chin. The benefits of chin augmentation are vast. A chin augmentation will accomplish several very important goals for many people. It can improve a person’s facial balance, structure and definition. These benefits alone are very significant for most people, young and old. The other benefits of chin augmentation consist of the anti-aging effects including correction of the jowl, droopy chin and looseness of the skin under the chin. These areas are addressed by placing a chin implant as this will place tension on the loose skin making it tighter. Therefore the chin implant procedure is one of the most effective techniques to make improvements to a person’s face.

The implants are made of a solid but soft silicone rubber that cannot be felt under the skin. The implant never moves and does not ever need to be replaced. Unlike breast implants that are made of liquid silicone inside a shell, there is no liquid in a chin implant therefore there is no risk of leakage of silicone. There are many different shapes and sizes of implants and it is up to the surgeon to know which style and size will look best for a given person. Everyone is different and therefore the implant that an individual receives must be carefully planned and thought out. Often the implant must be customized to fit the patient’s exact anatomy. I do an equal amount of chin implants on both men and women, both young and old. The procedure takes about 30 to 40 minutes and can be done under local anesthesia in the office. Chin augmentation is one of the most powerful techniques we have in our arsenal to improve one’s appearance and something that has been extremely beneficial for my patients over the years.

Fat grafting vs. fillers what works better?

I have had a lot of experience with fat grafting over the last 10 years and I have seen everything including fat lasting 10 years on some people and 6 months on other people. After thousands of my own patients and thorough discussions with many of my colleagues who I consider to be honest and at the top of the field. I have some strong opinions about the fat grafting craze. Our discussions have involved the topic of where we have been and where we are headed.

Fat grafting has been around for 40 years but it gained a lot of press and PR in the early 2000′s when the Plastic surgery reality shows hit the airwaves. It was a time when the economy was strong and there were doctors from all different specialties delving into the plastic surgery world the way pick axe salesman flooded the market at the time of the gold rush. It is well known that most of the doctors who advertise plastic surgery aggressively are not plastic surgeons at all. They are usually dermatologists and other Non Plastic surgeons who call themselves “cosmetic surgeons”. Fat grafting is a very safe procedure to do under local anesthesia and it does not require a scalpel. As a result of this fact, most of the “cosmetic surgeons” offer and advertise this procedure. As the plastic surgery boom occurred in the early 2000′s there was aggressive marketing and advertising by the cosmetic surgeons and they most often were advertising fat grafting. This created a buzz throughout the community and therefore most patients had heard of it and thought it would be the best option for themselves without knowing much about it. Now that the dust has settled and millions of patients have had fat grafting there is much less excitement about the procedure.

The reality is that the longevity of fat grafting is unpredictable. Some people will have long lasting results and others will not despite using the same technique and using the same amount of fat. There are certain things that are essential to do as the surgeon to make sure the fat has the best chance of surviving including low pressure harvesting and micro droplet technique injections. Still, the unpredictable nature remains. There are certain areas of the face where fat tends to survive like the tear trough. Areas where the fat tends to absorb quickly would be the lips. It is important to see a surgeon who you trust and who has a lot of experience with fat injections to get an honest idea of what to expect from the procedure. Fat grafting is still my favorite option for a few very select set of issues a patient may have but I use it much less frequently than I used to as patients often opt for more predictable and more permanent procedures.

Revision rhinoplasty is widely considered the most complex procedure to perform in the plastic surgery world and careful listening and planning and setting realistic expectations are essential to have a successful outcome and a happy patient. When presented with a patient my approach will depend on what occurred during the first rhinoplasty. Often times I will require the patient to send me the original operative report for me to review. This will allow to me to carefully plan weather I need to use septal cartilage or ear cartilage or rib cartilage or no cartilage to do the revision rhinoplasty. In the case that a patient can not acquire their operative report then I must rely on an extensive examination of the nose to try to determine what most likely took place previously and what will be required to fix the problems that the patient has been left with.

Revision rhinoplasty requires experience, talent and skill to perform well and it can be one of the most rewarding procedures for both patient and surgeon when done correctly.

Many patients ask what can be done to the tip of their nose when considering rhinoplasty. As a facial plastic surgeon and rhinoplasty specialist, the anatomy will dictate what is possible. However, depending on the shape I can increase or decrease the tip projection and increase or decrease the nasal length. The key elements for success when creating an aesthetically pleasing nasal tip are as follows. First, the patient’s desires must be fully understood because many people have different ideas and goals for their ideal nasal tip. The tip of the nose should be made symmetric and harmonious with the rest of the nose. Furthermore, the tip should be supported and stable to prevent any drooping, twisting or collapsing which is so prevalent with older style rhinoplasty techniques. The nasal tip is the most challenging and complex component to rhinoplasty surgery and mastery of this surgery takes a great deal of skill and experience. Refining the nasal tip is one of my favorite things to do and I consider this an art form and a passion.

K.L. is a woman in her 50’s who is a performer and relies on her nice appearance as well as her talent to enhance her career. She was feeling as though she appears aged over the last few years and she was hoping to have a full face rejuvenation. She was very concerned about looking weird or “done” but also didn’t want to go through surgery and not see any improvements. I planned a full facial restoration procedure that included mini browlift, facial fat grafting, deep plane anterior facelift and customized chin implant. I had to balance the degree of change that we created with her desire to look natural which is an art that I have honed over thousands of these exact same procedures.

These photos are 3 weeks after surgery. She still has some swelling and minor bruising that is easily covered with makeup. This is a perfect example of a dramatic change that doesn’t change who a person is, this change simply enhances or restores a person’s appearance back to what it was when they were younger.

I was presented W.B. who is a 56 year old man who had a facelift by a different doctor 1 year prior to his visit with me. He was not happy with his results and was looking for better definition to his neck and jawline as well as improvement to his scars around his ears.

He has a tell tale sign of a bad facelift called a “Pixie Ear” which is where the earlobe is stretched towards the angle of the jaw and this deforms the appearance of the ear and looks very unnatural. The reason that people will develop Pixie ears is because there is too much tension on the skin. This is a common problem associated with the older style facelifts that don’t tighten the SMAS layer which is the deeper muscle layer under the skin. I was able to significantly improve his neck and jawline as well as correct his Pixie ears with a revision “deep plane” facelift that placed all the tension on the deeper layers such that there was no tension on the skin layer upon completion of the surgery.

When addressing the face using liposuction it is important to use very small cannulas to ensure precise and problem free results. Liposuction to the neck is very safe as the neck is readily amenable to liposuction for many people. However, I see many people who come to me hoping to fix their neck after liposuction was done inappropriately on them. The jowls are a more treacherous place to perform liposuction as dents and divots are easily created even with small amounts of fat removal. The nasolabial mounds and cheeks are even more difficult to liposuction than the jowls.

The keys to micro liposculpture of the face are to be extremely judicious and precise when it comes to removing fat from the face and the use of very small cannulas is very helpful in limiting the problems that can readily occur if caution is not exercised.

As people age one of the most important factors that contribute to an old or tired appearance is loss of volume. This concept has only recently been widely understood and accepted by facial plastic surgeons. The loss of volume comes from loss of the cheek fat pad, loss of skin thickness and even thinning of the cheek bones. This volume loss leads to the midface tissue falling towards the floor and accentuates the laugh lines and the marionette lines and also contributes to jowl formation.

The correction of this problem will depend on each person’s particular anatomical findings. Treatment options include fillers, fat injections, midface lifting and cheek implants. All of these options have their place and all can be very effective if they are done on the right person.

I have seen many men recently who are interested in having upper and lower blepharoplasty otherwise known as upper and lower eyelid rejuvenation. This is a procedure that a person can have done on a friday and return to work by the following tuesday, in most cases. Men in general are less compliant than women with respect to their post operative course and typically want to return to activities earlier than I recommend. Upper and lower blepharoplasty is a combination of procedures that will allow for early return to activities and as a result is commonly requested over 3 day weekends.
