Rib Grafting for Function and Cosmetic Correction of Noses with Septal Perforation

Last week I performed perhaps the most difficult Revision Rhinoplasty of my career. This nice man had a history of nasal collapse secondary to a huge perforation of his septum from previous history of medications and chronic trauma to his nose. Septal perforations can be fixed if they are very small but the larger they are, the harder they are to fix. Septal perforations are irreparable if they become too large. To put this in perspective, a perforation the size of q-tip head is fairly easy to fix. A perforation the size of a dime has a very high rate of failure when correction is attempted and a perforation the size of a quarter is uncorrectable. This man had a perforation the size of a silver dollar. The reason that this is important is that the septum provides the support for the nose the same way the studs and beams inside the wall support the wall support the wall. Any trauma or insult to the septum puts the nose at risk of collapse which is why you see professional boxers with saddle nose deformities (because their septums become destroyed from repeated trauma). It is taught that someone has a septal perforation is not a candidate for rhinoplasty because the lining of the nose and perforation will weaken the support as I described above. I always have subscribed to that teaching until this particular person entered my office. This man has changed his life around and no longer does things that are detrimental to his nose or body. The appearance of his nose has severely affected his self esteem and is a daily reminder of the life and person he once was that he is trying to forget. He had been rejected for years by surgeons who he consulted with to fix his nose. I felt compelled to step outside the box if possible to help him move on with his life. He fully understood the risks associated with rhinoplasty and he desperately decided to proceed. I told him that I may be able to achieve a 5% improvement in his appearance and he thought that even that small amount would make him happy.

I performed a rhinoplasty using his own rib cartilage to rebuild his nose. During surgery, I proceeded very carefully step by step making sure not to violate the tiny amount of support that he had to his nose. At any step I was prepared to abort the mission if things looked too tenuous. As I proceeded I could not believe how much improvement I was able to get. I expected 5% improvement and I was looking at more like 95% improvement. Furthermore, I was able to increase his support by using rib cartilage so his nose is actually stronger following the rhinoplasty. He is now 1 week out from his revision rhinoplasty and I am astounded at the results! Never in my wildest dreams did I think I would be able to achieve this result. This is one of the most rewarding procedures that I have ever done and needless to say he is absolutely thrilled and amazed with his results.

I recently returned home from Dubai where I saw mostly Arab patients who wanted to have rhinoplasty and revision rhinoplasty. I saw many patients who had very distorted noses from previous rhinoplasty procedures in the Middle East. I did a lot of revision rhinoplasty on this trip and they were very difficult procedures but I am very pleased and proud of the results that I was able to achieve. The common theme among the patients who had bad rhinoplasty was the loss of support to the tip and the destruction of the structure of the bridge. The typical Middle Eastern or Arab nose has fairly strong tip support and good structure to the bridge and I needed to reconstruct these support mechanisms to recreate attractive appearing noses. Furthermore, the primary rhinoplasty that I did also had similar characteristics in that I reshaped the tips and the bridge without violating the support mechanisms. This will ensure that not only will the patients have outstanding and natural results but they will be permanent without the risk of collapse or distortion in the future. I am very fortunate to have had the experience that I have had and now I have an excellent understanding of the Arab or Middle Eastern anatomy and cultural dynamics that need to be understood to ensure that patients are more than happy with their results.

Your postoperative results are more likely to be consistent with patients who have thick skin.

Looking at before and after photos allows you to take note of the incision placement and scar quality in many instances. There are a few factors to note when reviewing before and after photos. It is important to make sure that both the before and after photo show the same angles and lighting. It is easy for a doctor to make the after photos look more flattering or artificially enhanced when angles and lighting are slightly manipulated. There are other subtle tricks that may be hard to detect which can artificially enhance the after photo so don’t forget to go to a doctor that you trust. Above all else, when deciding which doctor to choose you should pick a doctor who has a great reputation, and one who’s work you have seen. Let the before and after photos be an important guide but don’t let them be the only factor.

As a Rhinoplasty specialist in Beverly Hills I have taken care of people from around the world who are looking to improve the appearance of their nose with a rhinoplasty procedure. Often times this will include reducing the width of the nostrils or reducing the size of one nostril or lowering one or both nostrils to create a more symmetric and attractive appearing nose. Most often these asymmetries are the result of previous rhinoplasty but occasionally this can be the result of genetics. There are many different techniques to improve the appearance of the nostrils including removing excess tissue, adding tissue to lower or reshape a nostril as well suture techniques to change the nostril shape. Regardless of the technique chosen this is a very artistic endeavor that requires skill and judgment on the part of the surgeon. When done skillfully, the results can be excellent and can add the finishing touches to an already outstanding rhinoplasty result.

As a Beverly Hills Rhinoplasty and Revision Rhinoplasty expert, I see people of all different ethnicities who are seeking the Best Rhinoplasty surgeon they can find. The term ethnic rhinoplasty refers to anyone who is not of Caucasian descent. Regardless of ethnicity the principles and skill required are similar for every person to create an attractive and stable nose that will not collapse or develop problems over time. Often times we see thick skin in patients of African American and Asian descent but there are also many Caucasians with thick skin as well. It is now well understood that to improve the appearance of a nose with thick skin we need to push into the thick skin and this is most often done with cartilage grafting techniques to support the tip. These are the same principles and techniques that are often used in revision or secondary rhinoplasty as well.

The aesthetic goals of the patient need to be discussed thoroughly on a case by case basis because some people may want to maintain their ethnic (or family) look while others will want to eliminate it all together. I see about 60% of people who want to maintain their ethnic appearance to some degree and 40% of people who want to eliminate it. There is no right or wrong answer, it all depends on the patient’s desires. I really enjoy working with people of all ethnic backgrounds and now with my offices in NYC, Dubai, as well as LA, I have the privilege of doing a very high number of ethnic rhinoplasty procedures.

As a Rhinoplasty specialist in Beverly Hills I have had the pleasure of performing rhinoplasties on people of all different ethnicities including Latino, Middle Eastern, Armenian, African American and Asian. With the explosion of my practice in Dubai I have seen an even larger percentage of people who require ethnic rhinoplasty. I feel very fortunate to have the high level of experteise and experience that I have doing ethnic rhinoplasty so that I can give my Dubai patients the best results they could ever hope for right in their own backyard.

This procedure requires a very specializes set of skills that I have aquired over many years doing ethnic rhinoplasty in Beverly Hills. I have had many middle eastern patients fly to Beverly Hills and New York City to see me for ethnic rhinoplasty but now I am happy to be able to save them the travel expenses and deliver them the same Diamond quality rhinoplasty that people have become familiar with.

A.S. is a 54 year old woman who was interested in facial rejuvenation surgery. She is a very active person and is happily married. She has a very youthful spirit and has noticed over the last few years that her appearance does not match her personality. She feels that she looks much older than she feels and the she appears tired and mad when she is not. She was hoping for a fairly dramatic but natural change. A.S. is also unhappy with the appearance of her nose and her inability to breathe through her nose. She has had a previous rhinoplasty by a different physician several years ago and she still unhappy with the above mentioned problems with her nose. On examination I found her to have very flat and falling cheeks, hollowness under her eyes, a loose neck and jawline, severe wrinkles around her mouth and bulbous and drooping nasal tip with a deviated septum. I performed a fat transfer to her face, cheek implants, facelift, necklift, facial laser, revision rhinoplasty with deviated septum repair. She is healing very nicely and right on schedule.

Dr. Diamond,

It’s been two weeks since my procedures: face lift, neck lift, C02 laser resurfacing, fat injections, cheek implants, and a rhinoplasty. My husband asked me if I would do it all over again and I said, “Yes, in a heartbeat!” and I sincerely mean it. Before our consultation I had many concerns and doubts which is only natural when dealing with surgery of the face. However, you set my mind at ease by truly listening to my concerns and answering all my questions in a way that I could fully understand. You spent a significant amount of time and were never in a rush to answer my questions and explain the procedures. I came away feeling very confident in your abilities and I knew I could trust you. This allowed me to focus on your instructions for after surgery so that my recovery would be quick and the results positive. Despite being only two weeks post-op, I already am so happy with my results! I’m especially thrilled with how you repaired my deviated septum! After so many years, I can finally breath normally and my nose looks terrific.

What an amazing work family you have in Janet, Lynsay, and Ashley! These ladies were always respectful, caring, and knowledgeable. They took great care of me and were always there with a smile. I am truly happy with the outcome of all the procedures and so glad I took your suggestion to add cheek implants and the rhinoplasty. Thank you so much!

Sincerely,
A.S.

I have at least one Middle Eastern patient fly to Beverly Hills to have surgery with me every few weeks including many royal family members from different countries in that region. There has already been a lot of press coverage of my trip that has already hit the internet, but we are setting up several large press conferences to take place as soon as I arrive there. I look forward to spending time with my new friends and patients from the Middle East and I am really excited about this endeavor.

As a Beverly Hills Facial Plastic surgeon I am expected to give perfect or as close to perfect results as can be achieved and I take great pride when people tell me,”Dr. Diamond, you are the Best Plastic surgeon”. I saw a young man recently who had a previous rhinoplasty and he has been devastated ever since that procedure. He asked me to put a bump back on his bridge to recreate his previous “Jewish nose” as he put it. His nose had been severely scooped out so that it resembles a ski slope at this point. I perform many revision rhinoplasty procedures to correct ski sloped noses but most people want them straight. I have actually never been asked to put back a large bump or hump. I was not sure how effective I would be at recreating a hump. I used a nasal implant and added sheets of cartilage to the undersurface of the implant and sutured them all together. I was amazed at the result. I was able to give him a hump just as he asked for. This is not the best looking nose for his face by any means and not what I would have recommended had he asked me to what I think would look best. But this is a perfect example of making sure that the surgeon listens to the patients and gives the patient what they want, not what the surgeon wants. This has already tremendously boosted his self esteem and will continue to change his life for the better in many ways..
