I had the opportunity to meet a patient with the initials S.T. who is 55 years old and has chronic renal disease and therefore has been on chronic steroid therapy as well as other immune suppressing medication such as Myfortic and Remicade. One of the dreaded side effects of these medications is that the person’s face will became overly puffy and swollen in a very aggressive way such that there is significant distortion to the face.
This is called “Cushing’s syndrome” or “Cushingoid Face”. S.T. has suffered that problem and it has been devastating to her as it is for most people who suffer from it. It is common teaching that a person who is on steroids cannot have elective surgery because steroids can also inhibit the body’s ability to heal well, thereby increasing the risk of healing problems following surgery. This also pertains to the use of any immune suppressing medications like those mentioned above. I have had a lot patient’s who are on these medications for arthritis or lupus or any other number of over active immune system related disorders. With help from their rheumatologists I have safely operated on at least 10 people like this over the last several years. Prior to 3 years ago, I would simply decline to operate on these patients because I was taught that it wasn’t safe to do surgery on them). However, all of these patients were able to be off of all medications at the time of surgery and stay off their medications for 1 week after the surgery. I have never operated on anyone who was on their steroids during the surgery and the early post operative period. S.T. was not able to be off of her prednisone even for one day because of her kidney disease but she desperately wanted to look normal and therefore wanted the surgery. I spent a lot of time discussing this with her nephrologist who is very close with her as they have had a doctor patient relationship for many years. The nephrologist wanted S.T. to have the surgery to improve her quality of life. Based on S.T.’s desires and her motivation do this surgery and be very compliant with post operative care I decided to do her surgery for her. I did a facelift and I used a very safe technique called the Deep Plane Facelift. This facelift technique puts minimal tension on the superficial skin layer and most of the tension is on the deeper layers. This makes this the safest facelift technique for any patient who may have an issue with healing like S.T. Her nephrologist was involved in managing her kidney medications on a daily basis which included prednisone on the surgery day and every day afterwards. S.T.’s post operative healing course was perfectly normal without any problems at all. The results outstanding and she is so happy that she was able to have this surgery. S.T. and her husband feel that we were able to give her the appearance that she used to have before she suffered Cushing’s syndrome. I am very proud of S.T.’s courage and motivation to go through with this surgery and I will be more willing to take on patients with these types of problems in the future.

A.S. is a 54 year old woman who was interested in facial rejuvenation surgery. She is a very active person and is happily married. She has a very youthful spirit and has noticed over the last few years that her appearance does not match her personality. She feels that she looks much older than she feels and the she appears tired and mad when she is not. She was hoping for a fairly dramatic but natural change. A.S. is also unhappy with the appearance of her nose and her inability to breathe through her nose. She has had a previous rhinoplasty by a different physician several years ago and she still unhappy with the above mentioned problems with her nose. On examination I found her to have very flat and falling cheeks, hollowness under her eyes, a loose neck and jawline, severe wrinkles around her mouth and bulbous and drooping nasal tip with a deviated septum. I performed a fat transfer to her face, cheek implants, facelift, necklift, facial laser, revision rhinoplasty with deviated septum repair. She is healing very nicely and right on schedule.

Dr. Diamond,

It’s been two weeks since my procedures: face lift, neck lift, C02 laser resurfacing, fat injections, cheek implants, and a rhinoplasty. My husband asked me if I would do it all over again and I said, “Yes, in a heartbeat!” and I sincerely mean it. Before our consultation I had many concerns and doubts which is only natural when dealing with surgery of the face. However, you set my mind at ease by truly listening to my concerns and answering all my questions in a way that I could fully understand. You spent a significant amount of time and were never in a rush to answer my questions and explain the procedures. I came away feeling very confident in your abilities and I knew I could trust you. This allowed me to focus on your instructions for after surgery so that my recovery would be quick and the results positive. Despite being only two weeks post-op, I already am so happy with my results! I’m especially thrilled with how you repaired my deviated septum! After so many years, I can finally breath normally and my nose looks terrific.

What an amazing work family you have in Janet, Lynsay, and Ashley! These ladies were always respectful, caring, and knowledgeable. They took great care of me and were always there with a smile. I am truly happy with the outcome of all the procedures and so glad I took your suggestion to add cheek implants and the rhinoplasty. Thank you so much!

Sincerely,
A.S.

In the world of Plastic surgery in Beverly Hills I see men from all around the world who want to have their ”double chins” removed. The age ranges from late teenagers to upwards of 70 years old. For my younger patients often liposuction or a neck lift will do the trick. For those patients who have weak chins a chin implant will be the most powerful technique to correct their problem. For those older men who have loose skin a lower facelift may be necessary to chisel the neck and jaw line and give the definition to the lower face that they are hoping for. The best technique is really dependant on the given patients anatomy and I can best determine that with an in person consultation or very clear photos. The results that my male patients are receiving are so natural that they can be assured that they will not have any evidence of looking like they have had surgery. They will look great and their friends and acquaintances will not know why.

Addressing the jowls and neck with a mini facelift:

In the world of Plastic Surgery in Beverly Hills there are many different facelift terms that surgeons have developed to describe their techniques. Many of the terms that I hear of are gimmicky and marketing ploys to make the given procedure sound better than the competing doctor’s techniques. There is a popular television commercial that advertises “a breakthrough medical procedure” to describe their facelift. The procedure they are advertising is one of the oldest facelift techniques that has been around for over 50 years. The most important factor that will determine if your facelift will give you the results that you want is the skill level of your surgeon.

This brings us to the question,” Doctor, will a mini facelift be able to get rid of my jowls and tighten my neck?” To answer this question there should be a clear definition of what is a mini facelift. If you ask 10 different doctors to describe what their mini facelift technique are you will get 10 different answers. In general the neck is a fairly straightforward and easy area to tighten. Heavy jowls are more difficult to correct and therefore require a more aggressive procedure that I would not call a mini facelift but other doctors may. I use the term mini facelift to describe a very conservative procedure with a conservative dissection to achieve a conservative result. The mini facelift procedure, as I perform it, is the perfect technique for some patients and not very beneficial to others depending on anatomy. The best way to figure out which technique is best for you is to consult with a skilled Facelift specialist and let them guide you toward the procedure for you.

As a Beverly Hills Facelift and Rhinoplasty specialist I have taken care of people from over 100 different countries. Most recently, I did a full face rejuvenation on a man from Nigeria. This is the first time I have done surgery on a person from this country. He tells me that I am well known there based on my Dr. 90210 exposure.

His surgery included Facial Fat grafting, Upper and Lower eyelids, Endoscopic Browlift, Necklift, Facelift, and Revision Rhinoplasty. He had previously had some strange procedures done to his face by surgeons in France. They performed liposuction to his cheeks which is not supposed to be done because it can leave terrible deformities which it did on him. I was able to correct these problems. He also had very thin and receding hair, so I performed the Endoscopic Browlift through extremely small incisions that will not be noticeable even if his hair does recede beyond the incisions. It is a privilege to take care of people from such different backgrounds and I can only hope I see more nice people from Nigeria.

I am often asked “Doc am I to young to have a procedure done”? The answer to question depends on the person asking the question. Rhinoplasty surgery is often done on people 15 years old and up. Otoplasty or ear pinning is done on kids when they turn 5 or 6 years old (to prevent teasing in grade school).

Neck sculpting in the form of liposuction or even necklifts is often done on people in their early 20’s who were born with poor anatomy such that they don’t have the sculpted neck, chin and jawline that is so desirable and considered sexy and attractive by all.

Lower or minifacelifts as well are often done on people in their mid to late 30’s who have aged more rapidly than normal either based on lifestyle, major weight fluctuations or genetics. The fact is our knowledge and skill level is so advanced that skilled Facial Surgeons are able to deliver amazing and natural changes for people who are looking to improve their appearance and therefore age is not a major factor anymore.

I advise people that if they are bothered by something that I can correct for them in an excellent and safe way then they are not too old or young. People should only have cosmetic procedures done for themselves and not because other people think they should. A Person deciding to improve his or her facial appearance is no different than someone deciding to go on a diet or join a gym to improve the appearance of their body.

I have been doing more and more neck lift procedures for people with poor neck definition who do not want to have facelifts. The Necklift that I am performing first starts with liposuction to the remove the subcutaneous fat layer (which is the fat layer just under the skin). Then I am dissecting aggressively under the neck musculature to remove all of the excess subplatysmal fat (which is the fat layer that can not be removed with liposuction). I initially learned that this fat layer should not be removed because it is dangerous and as it could create significant hollowness under the chin. However, I have found, over the last 10 years, that this subplatysmal fat removal is the most powerful technique in my arsenal to sculpt a neck. I am able to create tight definition on even the most difficult of necks. There are people who are considered poor surgical candidates and many surgeons will not even do their procedure because the results will be minimal to nonexistent. However, with this maneuver I have been able to create dramatic contour and improvement for those people. As far as the problem of creating hollowness, this is not a issue at all because of the way in which the muscles are tightened up over the excised fat. The muscles are folded in to fill in the gap created by the excised fat so that the contour of the neck will be perfectly smooth. This technique has become my “ace in the hole” for almost all necklifts and facelifts to create the “movie star” neck and I believe this sets me apart from most, if not all of my competition.
