I have seen many men recently who are interested in having upper and lower blepharoplasty otherwise known as upper and lower eyelid rejuvenation. This is a procedure that a person can have done on a friday and return to work by the following tuesday, in most cases. Men in general are less compliant than women with respect to their post operative course and typically want to return to activities earlier than I recommend. Upper and lower blepharoplasty is a combination of procedures that will allow for early return to activities and as a result is commonly requested over 3 day weekends.

H.P. is a 56 year old man who was unhappy with the aged appearance of his upper and lower eyelids and wished to have upper and lower blepharoplasty to rejuvenate his eyelids. He also wanted a very subtle, natural and masculine appearance. He had an upper blepharoplasty and lower blepharoplasty with fat repositioning which is lower eyelid rejuvenation where the fat of the lower lids is preserved to prevent hollowness but repositioned to eliminate the baggy appearance. He was back to work 5 days after his surgery and had no discomfort after the surgery. This a very good representation of the expected results with male upper and lower blepharoplasty.

I am often asked if a browlift can and should be done at the same time as an eyelid procedure (blepharoplasty) or facelift procedure. As far as a browlift and facelift go, the two procedures have absolutely nothing to do with each other; therefore, the browlift will not affect the facelift and the facelift will not affect the browlift so there is no right or wrong answer. If the person in question wants to have his/her brows rejuvenated at the same time as the lower face rejuvenated, then there is absolutely no reason to do the two procedures at separate times.

Similarly, if the person only wants the brows rejuvenated then there is no reason to perform a facelift at the same time. The brows and upper eyelids have a much more intertwined relationship to each other. When the brows are lifted the upper eyelids will also slightly elevate and somewhat rejuvenated as well. There are some people who will have an excellent brow and eyelid rejuvenation by just having the browlift; therefore, that person would not require an upper blepharoplasty. There are those people who have extremely heavy upper lids that will not be rejuvenated enough with a browlift and those people will require a browlift and upper eyelid blepharoplasty done at the same time to get the result that they desire. There are those people who are in the middle, meaning that their brows and upper lids are only mildly heavy and those people require a surgeon who has a lot of experience if they are going to have the two procedures done at the same time. The novice surgeon should do the browlift first and the upper blepharoplasty at a later time. The skilled and experienced surgeon can safely perform them both at the same time. I perform browlift and upper blepharoplasty on upwards of 75% of my patients at the same time because most of my patients want to address both problems with one healing period.

Lower Eyelid Enhancement

As a Beverly Hills Facial Plastic Surgeon I take care of people from all over the world who want to improve their appearance. The lower eyelids are an important area to rejuvenate when trying to improve one’s overall appearance. It is important to make an accurate diagnosis of each person’s problem. Some people have bags on their lower lids, others have a sunken appearance, and others have a combination of both. For those who have bags, removing the fat pads is the appropriate procedure; however, I will admit that I see this problem in only a small percentage of patients. For those who are sunken I will fill their hollowness with fat taken from their own body (a “fat transfer”). I see this more often than isolated bags, but this is still not the most common variant. Most commonly, I see people who have both bags and hollowness. For these people I reposition their lower eyelid fat. This procedure reduces the bag but fills the hollow at the same time. This gives a very natural appearance that is youthful. This will give a permanent result and is in line with the goal of preservation of fat to prevent an aged appearance. The most youthful and beautiful faces have abundant fat, it’s just the fat is in the correct place. By repositioning fat we preserve volume and maintain a natural and youthful appearance.

As a Beverly Hills Facelift and Rhinoplasty specialist I have taken care of people from over 100 different countries. Most recently, I did a full face rejuvenation on a man from Nigeria. This is the first time I have done surgery on a person from this country. He tells me that I am well known there based on my Dr. 90210 exposure.

His surgery included Facial Fat grafting, Upper and Lower eyelids, Endoscopic Browlift, Necklift, Facelift, and Revision Rhinoplasty. He had previously had some strange procedures done to his face by surgeons in France. They performed liposuction to his cheeks which is not supposed to be done because it can leave terrible deformities which it did on him. I was able to correct these problems. He also had very thin and receding hair, so I performed the Endoscopic Browlift through extremely small incisions that will not be noticeable even if his hair does recede beyond the incisions. It is a privilege to take care of people from such different backgrounds and I can only hope I see more nice people from Nigeria.

This patient’s name is Lynn and she lives in Beverly Hills, California. She spent years searching the right Plastic surgeon to rejuvenate her face. Lynn came to see me about 3 months ago. I felt that I could give Lynn exactly what she was hoping for and she decided to have surgery with me. Lynn is now 6 weeks out from a Full facial rejuvenation that included a browlift, upper and lower eyelids, small chin implant, facial fat injections and rhinoplasty. Lynn is thrilled with her results and this has changed her life. She now has more self confidence then ever and she has seen that benefit spill over into her personal and professional life. This is just another example of how ones appearance can directly influence and benefit how they go about living their daily lives.

This is the first “testimonial” I have ever written. It is the first time I have ever been moved and excited enough to want to write one. But I have just had the most amazing experience. And it has to do with something I never thought I would do. I had plastic surgery on my face. I am going to start with the doctor because that is where it really all begins. His name is Dr. Jason Diamond. And he is incredible. His Beverly Hills office, The Diamond Face Institute, is located on Brighton Way at the corner of Canon Drive. It is a warm, relaxed state of the art facility and the staff is so kind and professional and they make you feel like you are the most important person in the world from the moment you first walk in. I will say more about them later. They are extraordinary. I have to say here that I had not been planning to have “work” done at that moment of my life. But the opportunity came up and I decided to do it, in a week and a half, not a lot of time to do any in depth research. I was so worried I would not find the right person to do perform the surgery. And then Dr. Diamond walked into the exam room. He is a tall, good looking man in impeccable scrubs, but the thing I was most struck by was his energy. It is palpable in the room and reassuring. I figured he had the stamina to stand on his feet for hours in an operating room and remain fully present. And then I looked in his eyes…they were blue and clear and deeply focused and I knew immediately that I could trust this man with my face. I was totally right. At the risk of sounding too dramatic, I have to say that Dr. Diamond is really a genius. His attention to detail combined with his incredible artist’s “eye” for proportion and the look of the whole, allows him to create results that are truely beautiful and seemingly just right for the individual patient he is working with. As he looked at my face that day I felt he really saw ME and that whatever he was about to suggest and to do would be with my unique and particular needs in mind. My biggest fear going into surgery was that I would not look like myself afterwards; and that I would have the “puffed and pulled” look so many people have after the swelling goes away. But Dr. Diamond made me look natural-even my most picky friends keep saying they can not believe how un-“done” I look. By the way, though I don’t know that much about the medical procedures involved, I have to say Dr. Diamond’s surgical expertise must be profound. My healing went so well and astoundingly quickly. One of the questions I often hear is “but where are the scars?” And they are almost impossible to detect-and were nearly invisible as early as 2-3 weeks into the recovery. By the way, I need to mention another astounding force of nature at the Diamond Face Institute-his nurse and right hand woman, Janet. She is the consummate professional, full of compassion and empathy and information, and so talented in all the procedures she herself performs. Janet redefines the words multi-tasking, and all this with grace and genuine caring for the patients. It has been 9 weeks since my operation. People that meet me for the first time seem to have no idea at all that I have had surgery. Since I know that I have, and because I am still basically a little insecure about being the new me, I hasten to tell them that “I just had my face done 2 months ago in case you are wondering” and every single one of them has said “you’re kidding…I would never ever have thought that”. One of the best tributes to Dr. D’s work happened in the elevator in his building as I was going up to the Penthouse suite about 2 weeks post op. A woman got on the elevator with me on the first floor and said “How do you stay so slim?”. I told her I had actually had surgery on my face 14 days ago and had lost 10 lbs. She asked what could I possibly have had done to my face since I was so young? I told her I was sixty two and that there had been quite a bit to do. Her jaw literally dropped and as she backed out of the elevator she sputtered “WHO was the doctor and WHERE can I find him?” I of course told her he was just a few floors away and his name was Dr. Jason Diamond. As the elevator door closed, I saw she was pulling a piece of paper and a pen out of her purse. I could go on and on with the compliments paid by others to this Dr’s work. But the important thing really is that I love how I look. When I look in the mirror I feel a confidence and a joy that I have not felt in many years. And this “from the outside in” approach is, I think, as valid as its opposite. It is why I am a sincere believer in plastic surgery for anyone who feels they want it. And if that is ever you, give yourself the gift of surgery with this outstanding doctor. I passionately believe there is no one else who brings such caring, dedicated, love for his craft to every patient he serves. Dr. Jason Diamond is unique and gifted and adores what he does. I feel so happy and lucky to have had the experience of his (yes, I’ll say it again!) genius.

Filling in Hollow Lower Eyes (Tear Trough Deformity)

As we age we lose volume to the face and one of the first places that this becomes noticeable is the lower eyes. This problem has been termed the Tear Trough Deformity. There are many methods to correct this problem and the appropriate method really depends on the patients given anatomy and goals. Filling of the tear trough can be very effective and noninvasive. I perform this procedure with Restylane or Juvederm or fat. The fillers are done in the office with local anesthesia and take about 10 minutes to do. There is no functional downtime and the person can go right to the gym but there may be a small amount of bruising or swelling that can last a few hours to a few days. The fillers last about 6 months. Fat injection to the tear trough is the most common method that I use to correct the tear trough because fat does the best job at filling the hollowness. This does require liposuction to harvest the fat so this procedure is done in the operating room. There usually is bruising or swelling for about 7-10 days with this procedure but the person can be back at work the next day if need be. Fat injections can be permanent although that is not guaranteed because the longevity is variable and unpredictable as everyone is different. There are 2 guaranteed permanent methods of filling the tear trough including lower blepharoplasty with fat repositioning and tear trough implants. The lower blepharoplasty is a very specialized and advanced type of blepharoplasty that corrects both the bags and the hollowness. The implants are made of a solid silicone and are placed from inside the lower eyelid. I perform all of these procedures very often and they are all excellent ways to refresh and rejuvenate the face. The appropriate technique will depend on one’s anatomy and goals and after talking to and examining a person I will be able to determine which technique is best for that person.
