In the Plastic Surgery world of Beverly Hills, skin care has always been a hot topic. I would like to explain my philosophy on skin care. My approach for people who have existing sun damage depends on the person’s desires.
I first try to ascertain if the person wants an aggressive or conservative approach. For those people who wish to have every little wrinkle removed I recommend an aggressive approach which consists of a Phenol peel or a CO2 laser. Both of these treatments will require general anesthesia and several weeks of healing time and can leave the skin red for up to several months (although makeup can be used for camouflage after 1 week). For those people who want improvements but do not want the downtime associated with the Phenol or CO2, I recommend the E-Matrix or Fractionated CO2 lasers which have both minimal downtime technologies. Because of this, a series of 3-5 treatments are recommended about a month apart. Both of these lasers will resurface and provide textural smoothing and improvement in sun damage. The treatments are done in the office with topical anesthesia. These lasers will improve the skin texture and tone and will correct many facial wrinkles but not to the extent of the Phenol peel or Co2 laser. Resurfacing procedures can be done at the same time as surgery or at a different time depending on each individual person.

I recently got the new fractionated CO2 laser and this is the best new technology on the market for non-ablative or no-downtime facial rejuvenation. This laser can improve wrinkles and the skin’s texture and tone and even provide a degree of tightening.

The treatments take about 20-30 minutes and are essentially painless. The downtime is very minimal although there may be mild redness and swelling for a few days. I am very impressed with the results that I have seen and plan to use this laser technology more and more now that I have seen first hand on my own patients what it can do.

I practice facial plastic surgery in perhaps the most affluent community in the world here in Beverly Hills and I, too, have seen a change in the demand for certain procedures during these trying economic times. During the recession, we have actually seen an increase in the demand for laser procedures and non invasive procedures such as liquid facelifts and in-office facelifts and in-office cheeklifts. This is understandable as people who may be looking for jobs cannot afford downtime if and when a job interview arises.

As a result I have become very involved in learning and helping to pioneer the latest and greatest in noninvasive technologies. The E-Matrix system by Syneron, Inc. is the latest in fractional skin resurfacing with radio frequency. I also frequently use the Pixel system by Alma which is the latest in fractional erbium resurfacing. Both of these systems allow for the improvement of fine lines and wrinkles, and refreshes the skin’s texture and firms the skin creating a youthful appearance. I have also been doing more IPL or fotofacials. This is a noninvasive technology to improve brown and red spots that are the result of age and sun damage. There has also been an increased demand for the Accent which is the latest technology for noninvasive body shaping and skin tightening. These technologies all allow for a painless and easy recovery with essentially no downtime.

The gold standard for resurfacing is still the CO2 laser and Phenol peel both of which I still do but not as often. These 2 techniques are the most powerful methods to eliminate facial wrinkles but both are associated with downtime anywhere from 2 weeks to several months and in today’s economic climate, that potential downtime is too long for many people.
